Contest Details
Simply upload your picture to enter! The contest is open to all patients, siblings, and parents.
- Only one photo allowed per entrant.
- Submit your best Mitchell & Bartlett smile picture for a chance to win the iPad!
- Be sure to attend our Patient Appreciation Day on August 6 at the Grasshoppers ballgame, when the prizes will be awarded!
How the contest works:
- Everyone will have 5 weeks to enter their photos starting on June 22.
- Beginning July 29th, all entrants will have 1 week to get as many people as they can to come to this site to vote for their entry.
- The entrant with the most votes wins the iPad.
- All entrants that attend the Patient Appreciation Day at the ballgame on August 6 will be entered in a drawing to win the $100 Best Buy gift card.
Contest Requirements
The contest is open to all Mitchell & Bartlett Orthodontics patients (past/present) and siblings and parents of Mitchell & Bartlett Orthodontics patients. Anyone under 18 years of age must have their parent or guardian's consent to enter. All photos must be owned by the entrant. Only entrants in attendance at the August 6th Patient Appreciation Day will be eligible to win the Best Buy gift card prize. Any votes that are deemed to have been cast by the same individual using multiple or "spoofed, temporary, anonymous, or disposable" email addresses is subject to disqualification.
